# Northlight Longevity > A physician led executive health and longevity practice operating from one clinic on an annual > membership. A membership year is five fixed stages across twelve months, with all measurement > concentrated into a single day. Cash pay, no insurance billing, no product revenue. One clinic, one record per member, and a named physician who holds that record for the year. The purchase is a membership year, not a package of tests and not a subscription to advice. The whole schedule of the year is published before payment. The practice is cash pay. It does not bill insurance and does not issue claims. Superbills are handled on request. It sells no supplements, infusions or devices, at the clinic or afterwards, and takes no product revenue of any kind. Prescribing is done by the member's physician when clinically indicated, and nothing is sold to the member at the clinic. Records belong to the member. They are exportable in full, at any time, including on leaving. The practice is not an emergency service. ## The membership year, stage by stage Dates run from the day of joining. Stated lengths are physician contact time, not time in the building. ### Stage 00. Intake and history Week 0. 60 minutes. Remote or at the clinic. Included in every tier. A physician takes the personal and family history: conditions, medication, sleep, work pattern, travel and how much of the year is spent away from home. The history is taken by a physician rather than collected by a form. Existing records are obtained, read and summarised back to the member before any visit to the clinic. Current medication and supplements are reviewed and a baseline blood pressure is recorded. Nothing else is measured at this stage, by design, so that the history is taken before a number is in the room. The member leaves with: their existing records read and summarised back, the written list of what will be measured at stage 01, and a dated appointment for the intensive. Measured at stage 00: history and physical examination, blood pressure, medication and supplement review. ### Stage 01. The diagnostic intensive Weeks 2 to 4. One day at the clinic, two if imaging is indicated. Included in every tier. All measurement is concentrated into one sitting so that the results describe a single day rather than a scattered year. The member arrives fasted. Order of the day: bloods first, then the physical examination, then cardiopulmonary exercise testing, then imaging if the physician has indicated it. The member goes home the same day. Nothing is interpreted in the corridor. Results are held until stage 02, when there is time to read them properly. Measured at stage 01, all ten measures: 1. Full history and physical examination 2. Blood chemistry and haematology 3. Lipoproteins and apolipoprotein B 4. Fasting glucose and insulin 5. Blood pressure, in clinic, with an ambulatory monitor fitted and worn after the visit 6. Cardiopulmonary exercise testing 7. Body composition 8. Sleep assessment, where indicated 9. Imaging, where clinically indicated, at the clinic or at a named partner facility 10. Medication and supplement review The member leaves with an ambulatory blood pressure monitor worn after the visit and a dated appointment to read the results. ### Stage 02. Findings Week 5. 90 minutes. At the clinic. Included in every tier. Nothing is measured at this stage. The physician reads the whole record with the member, in order, including the unremarkable results, because knowing what is normal is most of what the member came for. Where a result is uncertain or the evidence behind a test is contested, the physician says so at the time. The member leaves with a written plan stating what is being watched, what is being changed, what is being referred out, and what each subsequent measurement is actually for. Every item in the plan carries a stated reason. The plan is written to be legible to any other doctor the member shows it to, and it is the member's to keep. Referrals are made with records prepared in advance, and the measurement schedule for the rest of the year is fixed and dated here. All ten measures are reviewed at stage 02. None is re-measured. ### Stage 03. Quarterly reviews Months 4, 7 and 10. 45 minutes each. At the clinic or by telephone. Included from the Continuous tier upward. A defined subset is re-measured before each appointment, selected by the physician from what the intensive found, rather than repeating the full panel. Change is read against the member's own earlier numbers only, never against a population score. Medication and supplements are reviewed again, including anything the member started elsewhere. Typically re-measured at stage 03: blood chemistry and haematology, lipoproteins and apolipoprotein B, fasting glucose and insulin, blood pressure, body composition, medication and supplement review. History and examination are reviewed. Cardiopulmonary exercise testing, sleep assessment and imaging are not repeated at this stage. The member leaves with a written amendment to the plan, or a written note that no amendment was needed. ### Stage 04. Year close Month 12. 90 minutes. At the clinic. Included from the Continuous tier upward. The core panel is repeated under the same conditions as stage 01, so that the year has two ends and the comparison is like for like. Imaging is not repeated as a matter of course. Re-measured at stage 04: history and physical examination, blood chemistry and haematology, lipoproteins and apolipoprotein B, fasting glucose and insulin, blood pressure, cardiopulmonary exercise testing, body composition, sleep assessment where indicated, medication and supplement review. Imaging is not repeated. The member leaves with a written year summary covering what changed, what did not and what remains unresolved. The next year is agreed here, before anything renews. The full record is exported to the member on request, whether or not the membership continues. ## Which measures apply to a given member Which of the ten measures apply to an individual is a clinical decision made by that member's physician. It is not a fixed package. Continuous adds the three quarterly reviews of 45 minutes each, re-measurement between reviews where the physician indicates it, a named coordinator for scheduling and records, and the year close appointment with a written year summary. Directed adds extended physician time scheduled around the member's calendar, coordination and referral management with outside specialists, records prepared and sent ahead of any outside appointment, and travel scheduling for appointments handled by the practice. Programmes are defined pieces of work with a start and an end, taken alongside a membership or on their own. They run 8,000 to 19,000 USD depending on scope and on how much imaging and testing the work requires: cardiometabolic assessment, cognitive and sleep assessment, perioperative preparation, second opinion and records review. The current schedule is sent in writing before any commitment. Prices cover the practice's own work. Anything performed outside the clinic, such as imaging at a partner facility, is quoted in writing before it is booked and is never marked up. ## What the practice does not do These exclusions apply at every tier and are not negotiable at a higher one. - No supplements, infusions or devices are sold to members, at the clinic or afterwards. The practice takes no product revenue. - No biological age, longevity score or single index is issued. Results are read as results. - No outcome statistics are published, and no member is asked to trust one. - No patient testimonials, ratings, or before and after imagery are used. - No unsupervised home testing kit is sold as a programme. - No round the clock concierge medicine. The practice keeps business hours and says so. - No automatic renewal. The next year is agreed at stage 04 or the membership lapses. - No therapy is offered on the strength of its popularity. Where the evidence is thin, the physician says so at the time. ## Clinicians Every clinician at the practice is licensed and practicing. Full name, licence, board certification and training for each clinician are sent to a prospective member in writing before they book, and again in the welcome pack. The website holds roles rather than people, because a photograph and a first name are not credentials. ## Starting A 30 minute consultation with a physician from the practice, by telephone or at the clinic. There is no cost, no obligation, no form and no qualifying call with a salesperson. The prospective member sends one paragraph on why they are asking; the practice replies within two business days with times. Referrals are accepted from physicians and from current members, and are never required.
